ANNUAL CONFIRMATION OF PENSIONERS

In accordance with existing policies, a NAPOLCOM pensioner is required to update his or her profile and status for the continuation of pension payments.

Supporting documents shall include the following:

  • Signed Letter
  • Accomplished Annual Confirmation of Pensioner’s Form
  • Photocopy of LBP ATM Card
  • Photocopy of two (2) valid IDs
  • Sketch of Residence
  • Certification From:
    1. Barangay
    2. Institution
  • Medical Certificate
  • Death Certificate
  • Complete Physical Examination Report
  • Relevant laboratory or other diagnostic exam results
