ANNUAL CONFIRMATION OF PENSIONERS
In accordance with existing policies, a NAPOLCOM pensioner is required to update his or her
profile and status for the continuation of pension payments.
Supporting documents shall include the following:
- Signed Letter
- Accomplished Annual Confirmation of Pensioner’s Form
- Photocopy of LBP ATM Card
- Photocopy of two (2) valid IDs
- Sketch of Residence
- Certification From:
1. Barangay
2. Institution - Medical Certificate
- Death Certificate
- Complete Physical Examination Report
- Relevant laboratory or other diagnostic exam results
